Founded in 1976, the Thresholds Mothers Project provides comprehensive assistance to mothers with mental health challenges and their children. Parents in our program are working to provide safe, healthy and nurturing environments for their children, while also receiving support with mental illness, homelessness, and/or the effects of abuse, neglectDEAF PROGRAM
Deaf Program. The Thresholds Deaf Program is the first and only program of its kind in Illinois, offering comprehensive community-based services for persons with serious mental illnesses who are deaf or hard-of-hearing. Staffed by employees fluent in the use of American Sign Language (ASL) who understand the unique cultural needsof the deaf

TRANSITIONING FROM INSTITUTIONAL CARE Transitioning from Institutional Care. Thresholds was one of five initial providers selected by the Illinois Division of Mental Health (DMH) in 2012 to provide services to the class members of the Williams Consent Decree. In the first year, we transitioned 40% of the 256 class members that were identified for community reintegration. COMPREHENSIVE BEHAVIORAL HEALTH SERVICES (EMERGE) YOUTH & YOUNG ADULT SERVICES Comprehensive Behavioral Health Services The transition to adulthood is a challenging time for any young person, but especially so for those with serious mental illnesses. Emerging adults benefit from specialized support from people who truly understand this transitional phase as they navigate new challenges and responsibilities. Emerge was designed as EARLY INTERVENTION & FIRST-EPISODE PSYCHOSIS (MINDSTRONG YOUTH & YOUNG ADULT SERVICES Early Intervention & First-Episode Psychosis. MindStrong is a multistep care program that uses a multidisciplinary, evidence-informed approach to identify and treat young people at risk for serious mental health challenges or emerging psychosis symptoms. Early assessment and services are key in the return of young people to a healthy and fulfilling life. The Thresholds Research Department hosts, implements, and supervises evidence based, best practice intervention research studies in partnership with multiple collaborators from national and international universities and academic centers. We work together to develop and implement externally-funded research and training programs on behalf of the agency. TRANSITIONING FROM INSTITUTIONAL CARE Transitioning from Institutional Care. Thresholds was one of five initial providers selected by the Illinois Division of Mental Health (DMH) in 2012 to provide services to the class members of the Williams Consent Decree. In the first year, we transitioned 40% of the 256 class members that were identified for community reintegration. YOUNG MOTHERS (MOTHER’S PROJECT) YOUTH & YOUNG ADULT SERVICES Mothers Project. Founded in 1976, the Thresholds Mothers Project provides comprehensive assistance to mothers with mental health challenges and their children. UNEQUAL: BLACK ACCESS TO MENTAL HEALTH & SUBSTANCE USE Jails and prisons, where Black Illinoisans are incarcerated at nearly nine times the rate of white Illinoisans, have become the tragic alternative to treatment and affordable housing due to a lack of access to early mental health and substance use services, and the over-criminalization of drug use and poverty. The journey to self-sufficiency and recovery begins with a home. Thresholds residential programs offer a variety of community housing options for people with serious mental illness. A Thresholds residence is far more than mere shelter. Many residences also offer opportunities to develop social relationships and friendships through communal living and shared activities.HEALTH
An evidence-based practice (EBP) is a clinical intervention that has been consistently shown in research projects to assist consumers in achieving their desired goals of health and wellness. There are currently six EBPs that best fit the needs of our members. Research to date has shown that these methods are the best and most effective ways to increase consumer outcomes in these domains.HOPE
Thresholds rejects the notion that anyone is a lost cause. Our foremost goal is to ensure that every individual in our care, called members, receives the necessary skills and support to live an independent and fulfilling life. Recovery from mental illness is possible. Recovery is a process of finding and living a satisfying and meaningful life, as one defines it for oneself.
