_The Spine Journal_, the official journal of the North American Spine Society, is an international and multidisciplinary journal that publishes original, peer-reviewed articles on research and treatment related to the spine and spine care, including basic science and clinical investigations. It is a condition of publication that manuscripts submitted to _The Spine Journal_ have not been published, and will not be simultaneously submitted or published elsewhere. _The Spine Journal_ also publishes major reviews of specific topics by acknowledged authorities, technical notes, teaching editorials, and other special features. Letters to the Editor-in-Chief are encouraged.JOURNAL ACCESS

Productivity above replacement: a novel approach to assessing academic productivity in orthopedics and spine surgery Andrew J. Schoenfeld, Roman J. Schoenfeld DOI: https://doi.org/10.1016/j.spinee.2019.04.012 Vol. 19, Issue 8, p1305–1309 PreviewFull-Text HTML PDF xIn an era of increased financial constraints, academic programs are continuously challenged to improve the efficiency by which they deliver high-quality clinical care while also maintaining an educational mission and contributions to research . As a result, department chairs, financial administrators, and other medical center leadership are pressured to increasingly scrutinize the contributions of their faculty to the components of the academic mission . Although clinical productivity and participation in educational endeavors are fairly intuitive, effectively measuring academic productivity in an equitable way remains difficult .Clinical Studies

Supervised physical therapy vs. home exercise for patients with lumbar spinal stenosis: a randomized controlled trial Masakazu Minetama, Mamoru Kawakami, Masatoshi Teraguchi, Ryohei Kagotani, Yoshimasa Mera, Tadashi Sumiya, Masafumi Nakagawa, Yoshio Yamamoto, Sachika Matsuo, Yumi Koike, Nana Sakon, Tomohiro Nakatani, Tomoko Kitano, Yukihiro Nakagawa DOI: https://doi.org/10.1016/j.spinee.2019.04.009 Vol. 19, Issue 8, p1310–1318 PreviewFull-Text HTML PDF Supplemental Materials xExercise has been reported to improve short-term outcomes for patients with LSS in terms of disability and back and leg pain. However, no studies have compared supervised exercise with unsupervised exercise or quantified physical activity using a pedometer to confirm compliance with a home exercise program.*
The minimum detectable measurement difference for the Scoliosis Research Society-22r in adolescent idiopathic scoliosis: a comparison with the minimum clinically important difference Michael P. Kelly, Lawrence G. Lenke, Paul D. Sponseller, Joshua M. Pahys, Tracey P. Bastrom, Baron S. Lonner, Mark F. Abel DOI: https://doi.org/10.1016/j.spinee.2019.04.008 Vol. 19, Issue 8, p1319–1323 PreviewFull-Text HTML PDF xThe minimal clinically important difference (MCID) is the smallest change in an outcomes instrument deemed relevant to a patient. MCID values proposed in spine research are limited by poor discriminative abilities to accurately classify patients as “improved” or “not improved.” Furthermore, the MCID should not compare relative effectiveness between two groups of patients, though it is frequently used for this. The minimum detectable measurement difference (MDMD) is an alternative to the MCID in outcomes research.*

Reoperation rate after anterior cervical discectomy and fusion using standalone cages in degenerative disease: a study of 2078 cases Mootaz Shousha, Mohamed Alhashash, Hassan Allouch, Heinrich Boehm DOI: https://doi.org/10.1016/j.spinee.2019.08.003 Publication stage: In Press Accepted Manuscript PreviewFull-Text HTML PDF xOver the last two decades, there has been a rapid increase in the use of cervical spine interbody fusion cages. Reoperation rate remains an important determinant of procedural efficacy and safety.*
‘Cancer’
Christopher M. Bono
DOI: https://doi.org/10.1016/j.spinee.2019.08.001 Publication stage: In Press Accepted Manuscript PreviewFull-Text HTML PDF xCancer—it's a single word that, once it has fallen upon your ear, changes your life forever. As spine doctors, we are most often treating cancer within its last few blows to a patient's life in the form of metastatic disease. Metastatic—a term that conjures inevitable death—is synonymous with the nature of cancer itself in its quest to overtake normal tissue and convert it into a replicating mass of destruction. Treatment of metastatic disease is an exercise in catching up. We're running behind the train, trying to pick up the pieces falling off, knowing that we'll never stop it.*
Association between cervical degeneration and self-perceived non-recovery after whiplash injury Eric Rydman, Piotr Kasina, Sari Ponzer, Hans Järnbert-Pettersson DOI: https://doi.org/10.1016/j.spinee.2019.07.017 Publication stage: In Press Accepted Manuscript PreviewFull-Text HTML PDF xPre-existing radiological degenerative changes have not previously been considered a risk factor for non-recovery from neck pain due to motor vehicle accidents (MVA). Results from previous studies are however often based on assessment of plain radiography or MRI and little consideration has been given to facet joints. Furthermore, previous studies have often lacked a validated scoring system for degenerative changes.*
Physiological variations in the sagittal spine alignment in an asymptomatic elderly population Tito Bassani, Fabio Galbusera, Andrea Luca, Alessio Lovi, Enrico Gallazzi, Marco Brayda-Bruno DOI: https://doi.org/10.1016/j.spinee.2019.07.016 Publication stage: In Press Accepted Manuscript PreviewFull-Text HTML PDF xSagittal plane alignment is of crucial importance for the treatment of spinal deformities. When planning corrective strategies, spine surgeons need to refer to normative parameters’ ranges which characterize the alignment in the asymptomatic population. Changes are known to occur with age. For the Caucasian population, the reference ranges are extensively documented for the young and middle-aged adults. In contrast, only a few studies have evaluated individuals in advanced age (> 60 years) and in groups of limited numbers of subjects(less than 50).

The Effects of Varenicline on Lumbar Spinal Fusion in a Rat Model Jason Kang, Juliane D Glaeser, Brian Karamian, Linda Kanim, Zachary NaPier, Jayme Koltsov, Timothy Thio, Khosrowdad Salehi, Hyun W Bae,Ivan Cheng
DOI: https://doi.org/10.1016/j.spinee.2019.07.015 Publication stage: In Press Accepted Manuscript PreviewFull-Text HTML PDF xSmoking is detrimental to obtaining a solid spinal fusion mass with previous studies demonstrating its association with pseudoarthrosis in patients undergoing spinal fusion. Varenicline is a pharmacologic adjunct used in smoking cessation which acts as a partial agonist of the same nicotinic receptors activated during tobacco use. However, no clinical or basic science studies to date have characterized if varenicline has negative effects on spinal fusion and bone healing byitself.
