REQUESTING YOUR MEDICAL RECORDS If medical records must be sent to another clinician, referred to by Sheppard Pratt, a patient will be prompted to sign the appropriate release of information form prior to medical records being shared. The Crisis Walk-In Clinic serves people of all ages and is located within the crisis suite within the Weinberg Building of the Towson Campus. REQUESTING YOUR MEDICAL RECORDS If medical records must be sent to another clinician, referred to by Sheppard Pratt, a patient will be prompted to sign the appropriate release of information form prior to medical records being shared. The form can be obtained by calling the Heath Information Management Department at 410-938-4219, or can be downloaded by clicking here.VETERANS HOUSING

Thriving Germantown is a wide ranging multi-generational, multi-tiered case management and care coordination program developed to reduce the impact of poverty for families living in the Germantown area, with emphasis on the Daly Elementary School community. Thriving Germantown uses an approach that builds partnerships to create a broad EXTENDED TREATMENT HOME: RUXTON HOUSE Ruxton House. Ruxton House is an eight-bedroom residence located in a beautiful, quiet neighborhood near The Retreat.
