Prince Edward Family Health Team: Nurse Practitioner The Prince Edward Family Health Team (PEFHT) is seeking a Nurse Practitioner (NP) to join our team on a full-time, permanent basis. The PEFHT provides a single point of access to health care services for County residents. Our interdisciplinary team offers comprehensive patient-centred, primary health care including diagnosis, treatment and management, prevention, rehabilitation, palliative care and community health promotion. The NP role has been developed over the past 10+ years by the NPs and their collaborative partners within the PEFHT. NPs engage in autonomous decision making and authority within their scope and shared decision-making when it is beyond the NP Scope of Practice. NP clients include individuals, families and the community as a whole. There is a focus on caring for our most fragile, often including our 65+ population. NPs provide safe, effective and ethical care. Within the context of a client-centred care model and working within a multidisciplinary team, the NP will: provide shared care for their patients along with their physician partners; assess, investigate, diagnose and treat patients across the lifespan, utilizing critical thinking and evidence-based medicine and nursing as a guide to practice; make referrals independently to physician specialists as per CNO Standards of Practice; provide expert coaching, guidance, and counseling - e.g. patient education, supportive counseling, options counseling, grief counseling, CBT if qualified; obtain and maintain QHC Hospital privileges to facilitate continuity of care and have the authority to order hospital-based tests within their scope of practice; monitor patients EMR for test results and consultants’ notes; provide service encounter statistics to MOH as requested by the NPAR Program; consult with multidisciplinary team members, physicians and other health care professionals to achieve excellence in client-centred care. lead or participate in PEFHT patient-centred programs, e.g. Collaborative Care Program, which requires home visits, assessment, and working collaboratively with multiple community agencies. Qualifications: BScN (MScN an asset). Registration in the Extended Class with the CNO. Member of RNAO or NPAO and carry liability insurance. Excellent verbal and written communication, facilitation and interpersonal skills; ability to build and nurture positive working relationships both within and outside of the PEFHT. Knowledge of health care facilities and community health services, social agencies etc. Demonstrated ability to work independently as well as collaboratively within a multidisciplinary team, and within a brisk work setting. Proficient with electronic medical records and standard office software. Valid driver’s license, access to a reliable vehicle, willingness to provide home visits. Simcoe County: Associate Director of Resident Care The County of Simcoe is an upper tier municipality located north of the Greater Toronto area and comprised of 16 towns and townships and including the cities of Barrie and Orillia. The largest county in Ontario based on population (480,000), the County offers a range of programs and essential services, including social housing, land ambulance, emergency planning, environmental services, roads maintenance, Ontario Works, children’s services, long term care and seniors services, a library co-operative, museum, archives, forest management, tourism, GIS and land use planning. Overview The Associate Director of Resident Care (ADRC) is responsible for the daily planning and co-ordination of resident care in a 150 bed nursing home with 215 staff in Collingwood, including direct supervision of nursing staff, infection control monitoring, and providing leadership in health and safety. The ADRC assists the Director of Resident Care in supervising of nursing and personal care services to ensure that an outstanding level of resident care is maintained and enhanced on a continuous basis in accordance with the policies of the County and applicable legislation. As a valued member of Simcoe County’s Long-Term Care and Seniors Services division the ADRC demonstrates a commitment to resident safety by providing quality care in accordance with the organization’s mission, vision, and values. Key Responsibilities Supervises and assists in the development and co-ordination of a comprehensive needs-based plan of care developed through such venues as admission, annual, and special care conferences, resident and family meetings and ongoing consultation. Assist with the admission, discharge, and transfer processes for all residents of the Home. Assists the Director of Resident Care in the management of an employee Attendance Management Program for the Nursing Department to ensure effective human resource management and day to day operations. Monitors nursing and personal care services to ensure they meet all applicable legislation. Assists the Director of Resident Care in maintaining an organized program of nursing services to meet resident’s nursing and personal care needs consistent with the professional standards of practice of the College of Nurses and the Ministry of Health Long Term Care Program Standards. Assists in the delegation of clinical and supervisory responsibilities to appropriately qualified staff, according to the applicable legislation and standards of the College of Nurses of Ontario. Develops and monitors a facility-wide infection control program, provide infection surveillance and outbreak management activities. Works collaboratively with multi-disciplinary teams. Ensures that information obtained by nursing staff is documented appropriately and is available to relevant members of the health care team, resident, and family in a professional manner, consistent with facility policies and procedures for maintaining confidentiality of information. Purchases equipment and supplies according to needs, budget limitations and purchasing policy as approved by the Director of Resident Care. Assists in the development and implementation of Nursing programs and related policies and procedures. Develops and maintains effective relationships with residents, families, the public, management, other service providers, committees, organizations, institutions, and others. Promotes co-operation of services/programs with community agencies and other health care providers. Provides leadership and supervision to Nursing Department staff in a manner that motivates, guides and directs employees to the realization of departmental goals and objectives. Maintain a work environment that promotes staff participation, teamwork and positive labour relations. Reviews and approves staff scheduling as required. Facilitates orientation and training programs and oversee day-to-day activities of staff according to County policies and guidelines. Participates in and demonstrates an understanding of resident safety principles and practices into all day to day activities. Follows all safe work practices and procedures and immediately communicates any activity or action which may constitute a risk to resident safety. Assists in the recruitment, evaluation, discipline, and termination of personnel. Prepares and presents written reports and participates in committees as required. Assumes responsibility of Resident Care Department in the absence of the Director of Resident Care. Works in compliance with the Occupational Health & Safety Act and the Long-Term Care Homes Act, and their regulations in performing duties in a safe manner and follows all County of Simcoe Health & Safety policies, procedures and legislation. Maintains confidentiality in accordance with the Municipal Freedom of Information and Protection of Privacy Act and the Personal Health Information Protection Act. Required Experience, Education, Technical Skills, and Certification 3-5 years’ experience in progressively responsible, related positions, preferably in a long-term care setting with 2 years in an Associate Director of Resident Care, Nurse Manager or similar position. Experience in the supervision of staff and the administration of employment policies and collective agreements. Strong knowledge of health care principles obtained through the completion of a diploma and/or degree in Nursing, with additional education or equivalent in the care of seniors. Current registration with the Ontario College of Nurses as a Registered Nurse. Strong understanding of policies and related legislation or initiatives and their significance and potential impact. Very good leadership and supervisory skills and ability to interpret and apply employment policies and collective agreements. Strong interpersonal and communication, planning, organizational, time management and multi-tasking skills. Knowledge of word-processing and spreadsheet application skills.
