xIn the multicenter, phase 3, BLINDED FOR REVIEW, hypofractionated radiotherapy (HF) was compared with conventionally fractionated (CF) radiotherapy. In previous reports, we could not demonstrate the postulated superiority of hypofractionation in terms of relapse-free survival at 5-year. The frequent use of long-term androgen deprivation therapy might have had substantial effects on relapse-free survival. xSummary SABR is increasingly used in recurrent/previously irradiated H&N cancers and requires high degrees of accuracy and reproducible set-up. We analyzed 405 HN-SABR fractions using our cushion-mask-bite-block immobilization with ExacTrac x-ray and CBCT to determine set-up and positioning accuracy. Differential error magnitudes suggest a one-size-fits-all PTV margin may not be optimal in HN-SABR, where 2mm expansions may be adequate for skull base targets, but increased margins for neck and mucosal targets (3mm) maybe necessary.
