Hi Nick, thanks for your question. I agree it is a bit of a nuance. Early on when Stuart and I are discussing miscarriage, we are stating that after miscarriage is already confirmed (i.e.:Diagnosed US findings, smaller than expected CRL, no/slow FHT, partial POC expelled etc) miscarriage is unfortunately inevitable, and there is little that can be done to "prevent" pregnancy loss. To perform a thoracentesis, first position your patient sitting at the edge of the bed with their arms resting on a table in front of her (if tolerated). Use ultrasound in the midscapular line to locate the superior border of the effusion, and mark the site 1-2 rib spaces lower. This seems to work in my clinic practice for ace-inhibtor agioedema. Could I have actually witnessed the effects of time and drug cessation instead of true benefit from TXA?
